Public Notifications

Generally, complaints and investigations are confidential. We are, however, required under the Health Professions and Occupations Act to publish certain outcomes arising from complaints, investigations and administrative matters (such as summary protection orders, disciplinary orders and requests to the Director of Discipline for citation). The public notifications below are also reflected in the public register. 

Disciplinary orders will be listed on the website for ten years but remain on the public record permanently. Please use our contact form to obtain disciplinary information older than ten years. 

Publications listed prior to April 1, 2026 concern disciplinary action taken by the former discipline committee, consent orders or other action taken in relation to a serious matter, under s. 39.9 of the Health Professions Act, which is no longer in force. 

Media reporting on these notices should provide the full name and location of the oral health professional in order to avoid confusion with any other former or current oral health professional who may have a similar name or initials.
